The Delhi Belgian Club is in the process of winding down all financial transactions and therefore, requires all shareholders to submit their share certificates for redemption.
Please sign the back of your original share certificate and complete the required form, giving us instruction as to what we are to do with the rebate you are owed.  Your options are – (a) - pay you directly - or – (b) - donate the proceeds to the Norfolk Community Foundation for future use as community donations, in the name of “The Delhi Belgian Club”.
The SIGNED share certificates and accompanying forms, are to be dropped off at, or mailed to:
Brimage Law Office – 160 Main Street, Delhi, ON, N4B 2L9
The required form can be picked up at Brimage Law Office in Delhi, downloaded at www.belgianhall.ca or can be obtained by email at contact@belgianhall.ca.  
This form also includes instructions for lost share certificates and/or dealing with shares for deceased family members.
YOU WILL NOT RECEIVE YOUR PAYMENT IMMEDIATELY – so PLEASE include your current, CORRECT mailing address so that a cheque can be mailed to you once the share(s) have been processed (this will take approximately 4 weeks).
